What Bedsores Are & Why They Form

A bedsore, also called a pressure ulcer or decubitus ulcer, is an injury to the skin and underlying tissue caused by sustained pressure cutting off blood flow to an area. They form most often over bony prominences: the tailbone, hips, heels, and elbows. For residents with limited mobility, the risk is constant, and the responsibility for prevention falls on the facility’s staff.

The Four Stages of Pressure Ulcers

Pressure ulcers are classified into four stages. Stage 1 presents as redness on intact skin. Stage 2 involves a shallow open wound or blister. Stage 3 extends into the fatty tissue beneath the skin. Stage 4 is a full-thickness wound with exposed muscle, tendon, or bone. A wound that reaches Stage 3 or 4 can reflect a serious failure of care, not an unavoidable outcome.

Care Failures That Allow Bedsores to Develop

Most pressure ulcers in nursing home settings are preventable. Recognizing how they develop can help families identify whether neglect played a role.

Infrequent repositioning: Residents who can’t reposition themselves generally require a scheduled repositioning protocol to relieve pressure and maintain blood flow. When that schedule isn’t followed, tissue breaks down.
Prolonged moisture exposure: Contact with sweat, urine, or feces weakens the skin barrier and significantly increases the risk of breakdown and infection.
Malnutrition and dehydration: Inadequate nutrition and fluids slow the body’s ability to repair damaged tissue and leave residents more vulnerable to pressure injuries.
Understaffing: When facilities operate with too few staff, residents go longer without repositioning, hygiene care, and skin monitoring. Understaffing is a recognized and documented factor in nursing home neglect claims.
